College Information and Scholarship Survey

This form must be filled out, even if you have given us your acceptance and award letters. Fill in all the parts of this form. Complete this no later than June 1st.

June Senior Checklist

In order for you to pick up your diploma, the following conditions must be met by June 1st, 2010. Bring Information to the College Office in Room 424.

  • You must submit your College Information and Scholarship survey which can be completed below.
  • You must submit a stamped legal size envelope addressed to the college you will be attending so that we may send your final transcript. Write your name and official class under the flap.
  • You must have a clear book account. You cannot owe textbooks, library books or College Now books. Check to see if you have any outstanding books owed by seeing Mr. Maldonado for textbooks (4th floor Book Room), Mr. Trowbridge for Library books (Library) or Ms. Campos/Ms. Chiarello for College Now books (Room 112B).

Remember: Once you graduate, some of your records are moved to the record office. As a result, it could take at least a week for your college to obtain official documnts in the fall so please be sure to complete everything before you leave.

Recommendations will no longer be kept on file due to limited storage space. If you need a transcript once you have graduated, you must see Ms. Drori in Room 211. The college office does not provide transcripts once a student has graduated.

The college advisor does not provide college advisement for transfer applicants. If you feel that you may transfer out of your selected college before your freshmen year is completed, please make an appointment with the college advisor before graduation to find out the proper procedures ahead of time. Also, no visitors are allowed in the college office without an appointment once you have graduated.
